The Geriatrics program is a cornerstone program at Queensway Carleton Hospital. The program’s services deliver acute and sub-acute clinical care to individuals aged 65 and older who are encountering shifts in their functionality, memory, emotional state, or managing complex medical conditions, all of whom could benefit from a thorough geriatric evaluation. Our program extends its interdisciplinary services to inpatient, outpatient, and community settings.

 

Patients are directed to the Geriatrics program for an evaluation encompassing a range of issues including:

  • Recent variations in physical, cognitive, or functional capacities, such as falls, diminished mobility, dementia, delirium, shifts in mood, and reduced self-care capability.
  • Significant shifts in the level of assistance required, caregiver strain, and the contemplation of moving to an institutional setting.
  • Safety concerns, including physical, emotional, social, and environmental aspects.
  • Unexpected escalation in healthcare service utilization within the past six months.
