Queensway Carleton Hospital has an Inpatient Geriatric Consult Team which includes two Geriatricians, a Clinical Nurse Specialist in Geriatrics, and a Registered Nurse in Geriatrics. The team provides comprehensive assessments to hospitalized geriatric patients who have and not limited to:
The Inpatient Geriatric Consult Team will:
- Conduct comprehensive geriatric assessments.
- Provide recommendations for care planning.
- Support and education to staff, patients, and families.
- Clinical follow-up and reassessment.
